By virtue of an affidavit sworn before the Notary Public, Cuttack on the 13th November 2023, I, Ayesha Ahmed, W /o Mohammed Aaves Ahmed, At Moulavi Mohallah (Nandol), Post. Mahasinghpur, Dist. Cuttack have changed my name from "Rajashree Sahoo'' to "Ayesha Ahmed''. Henceforth, I shall be known as Ayesha Ahmed for all purposes.
