By virtue of an affidavit sworn before the Executive Magistrate, Darpan, Dhanmandal, Jajpur on the 26th December 2023, I, Chinmayee Bal, W/o Biswajit Bal, At Majhipada, P.O. Achhutabasanta, P.S. Balichandrapur, Dist. Jajpur, Odisha, PIN Code-754205 have changed my name from "Chinmayee Pradhan " to "Chinmayee Bal''. Henceforth, I shall be known as Chinmayee Bal for all purposes.
