By virtue of an affidavit sworn before the Notary Public, Puri on the 7th September 2023, I, Saraswati Mallik, aged about 22 years, D/o Gheneswar Mallik, resident of Vill. Bhattapur, Badabilasahi, P.O. Baral, P.S. Satyabadi, Dist. Puri-752046, Odisha declare that, I have changed my name from "Saraswati Mallik " to "Laxmipriya Mallik''. Henceforth, I shall be known as Laxmipriya Mallik for all purposes.
