By virtue of an affidavit sworn before the Executive Magistrate, Bari on the 29th February 2024, I, Asirbad Barik, S/o Sadasiva Barik, Vill. Arangabad, P.O. Bari, Cuttack, P.S. Bari Ramachandrapur, Dist.Jajpur, Odisha declare that, I have change my name from "Ashish Kumar Barik " to "Asirbad Barik''. Henceforth, I shall be known as Asirbad Barik for all purposes.
