By virtue of an affidavit sworn before the Executive Magistrate, Berhampur, Ganjam on the 1st February 2024, I, G. Debaki Rao, W/o Shri G. Keshab Rao and D/o Satrughana Reddy of village Ankuli, P. O. Berhampur, P. S. B. N. Pur, Dist. Ganjam, PIN-760010, Odisha declared that, I have changed my name from "L. Jamalu Reddy'' to "G. Debaki Rao''. Henceforth, I shall be known as G. Debaki Rao for all purposes.
