By virtue of an affidavit sworn before the Executive Magistrate, Bhubaneswar, on the 6th February 2024, I, Meera Padhi, D/o Ram Krishna Padhi, Flat No. 307, Manohar Residency (Near SBI Branch), Kesora, Bhubaneswar, Dist. Khurdha, Odisha-752101, declare that, I have changed my name from "Uma Parbati Padhi'' to "Meera Padhi''. Henceforth, I shall be known as Meera Padhi for all purposes.
