Balianta Talagarh, P.O./P.S. Balianta, Dist. Khurda, State Odisha, PIN-752101, declare that, I have changed my surname from "Pravat Kumar Sahoo'' to "Pravat Kumar Mohapatra''. Henceforth, I shall be known as Pravat Kumar Mohapatra for all purposes.
