By virtue of an affidavit sworn before the Notary Public, Bhubaneswar on the 20th January 2024, I, Dinesh Kumar Swain, Vill. Raghunathpur, P.O. Ghiakhla, P.S. Balugaon, Dist. Khordha (Odisha), So Indramani Swain declare that, I have changed my name from "Dinesh Swain'' to "Dinesh Kumar Swain''. Henceforth, I shall be known as Dinesh Kumar Swain for all purposes.
