By virtue of an affidavit sworn before the Executive Magistrate, Bhubaneswar on the 22nd February 2024, I, Pradip Kumar Das, S/o Late Ramachandra Das, Plot No. 291, Tapobana Sahi, near Sai Mandir, P.S. Khandagiri, Bhubaneswar-751030, Dist. Khordha, Odisha declare that, I have changed my daughter's name from "Tikili Das'' to "Pratyasha Subhadarshini Das''.
Henceforth, she shall be known as Pratyasha Subhadarshini Das for all purposes.
