Chandrasekharpur, P.O./P.S. Chandrasekharpur, Bhubaneswar, Dist. Khorda, Odisha, declare that, I have changed my name from "Donki Dilip Kumar Patra'' to "D Dilip Kumar Patra''. Henceforth, I shall be known as D Dilip Kumar Patra for all purposes.
