Brahmanmara (Behind Tahshil Office), Birmitrapur, P.O./P.S. Birmitrapur, Dist. Sundargarh, Odisha, PIN-770033, declare that, I have changed my name from "Martha'' to "Mantej Pardesi''. Henceforth, I shall be known as Mantej Pardesi for all purposes.
