By virtue of an affidavit sworn before the Notary Public, Cuttack on the 14th March 2024, I, Narendra Patra, S/o Harihar Patra, At/P.O. Bodasa, P.S. Khandapada, Dist. Nayagarh, Odisha-752077 have changed my name from "Jai Patra'' to "Narendra Patra''. Henceforth, I shall be known as Narendra Patra for all purposes.
