By virtue of an affidavit sworn before the Notary Public, Puri on the 17th February 2024, I, Jyostna Rani Singh, D/o Biswamohan Singh, At Ramachandi Sahi, Near Laxmi Talkies, P.S. Puri Town, P.O./Dist. Puri, Odisha, declare that, I have changed my surname from "Jyostna Rani Sinha'' to "Jyostna Rani Singh''. Henceforth, I shall be known as Jyostna Rani Singh for all purposes.
