By virtue of an affidavit sworn before the Executive Magistrate, Bhubaneswar on the 6th February 2024, I, Kalayani Mahapatra, D/o Trinath Mohapatra, R/o Sahanikera, P.O. Ghanapana, P.S. Puri Sadar, Dist. Puri, Odisha, declare that, I have changed my name and surname from "Kalyani Mohapatra'' to "Kalayani Mahapatra''. Henceforth, I shall be known as Kalayani Mahapatra for all future purposes.
