By virtue of an affidavit sworn before the Executive Magistrate, Cuttack, Affidavit No. 1571, dated the 16th April 2024. I, P.M Shaktiswarup Rout, At/P.O./P.S. Nischintakoili, Dist. Cuttack, Odisha-754207. I have changed my name from "P.M Shaktiswarup Rout'' to "Shaktiswarup Rout''. Henceforth, I shall be known as Shaktiswarup Rout for all purposes.
