By virtue of an affidavit sworn before the Notary Public, Cuttack on the 23rd December 2023, I, Mrs. Arundhati Choudhury, Address : At Bamphisahi, Post Telenga Bazar, P.S. Purighat, Dist. Cuttack have changed my name/surname from "Arundhati Das'' to "Arundhati Choudhury''.
Henceforth, I shall be known as Arundhati Choudhury for all purposes.
