By virtue of an affidavit sworn before Notary Public, Khordha on the 5th April 2024, I, Sunita Paikray, W/o Sumant Paikray, Vill./P.O. Haja, P.S. Jankia, Dist. Khordha, Odisha declare that, I have changed my name from "Sunita Samantaray'' to "Sunita Paikray''. Henceforth, I shall be known as Sunita Paikray for all purposes.
