Suryanarayan Achari, Plot No. 78, near Budheswari Colony, Laxmisagar, Bhubaneswar, Dist. Khordha, Odisha, PIN-751006, declare that, I have changed my name/surname from "Anupoju Kusuma'' to "M. Kusuma Achari''. Henceforth, I shall be known as M. Kusuma Achari for all purposes.
