RRL, P.S. Nayapalli, Bhubaneswar, Dist. Khordha- 751013, Odisha, declare that, I have changed my daughter's name from "Anannya Nayak'' to "Smita Sahoo''. Henceforth, she will be known as Smita Sahoo for all purposes.
