By virtue of an affidavit sworn before the Executive Magistrate, Bisra vide Sl. No. A/600/ 27- 06-2024, dated the 27th June 2024 I, Manisha Madhulika Mohapatra, Address At Purukhi, P.O. Singiri, P.S. Remuna, Dist. Balasore, Odisha declare that, I have changed my name from "Manisha Madhulika Hembram'' to "Manisha Madhulika Mohapatra''. Henceforth, I shall be known as Manisha Madhulika Mohapatra for all purposes.
