By virtue of an affidavit sworn before the Executive Magistrate, Angul on the 23rd February 2024, I, Purusottam Naik, S/o Late Uchhab Naik, Vill. Antulia, P.O. Dandasingha, P.S. Purunakote, Dist. Angul, Odisha, declare that, I have changed my son's name from "Prashanta Kumar Naik'' to "Prashant Kumar Naik''. Henceforth, my son shall be known as Prashant Kumar Naik for all future purpose.
