By virtue of an affidavit sworn before the Executive Magistrate, Sadar, Cuttack, on the 5th January 2024, I, Suchismita Bhoi, W/o Nirmal Bhoi, Vill./P.O. Bodhapur, P.S. Kishannagar Dist. Cuttack, Odisha, declare that, I have changed my name from "Tikili Das'' to "Suchismita Bhoi''. Henceforth, I shall be known as Suchismita Bhoi for all purposes.
