By virtue of an affidavit sworn before the Notary Public, Cuttack on the 19th July 2024, I, Narottam Dash, At OTM Colony, P.O./P.S. Choudwar, Dist. Cuttack, Permanent residence At Kantol, P.O./P.S. Athagarh, Dist. Cuttack have changed my name/surname from "Nanda Kishore Panda'' to "Narottam Dash''. Henceforth, I shall be known as Narottam Dash for all purposes.
