By virtue of an affidavit sworn before the Notary Public, Cuttack on the 31st May 2024, I, Mahendranath Sahoo, S/o Late Mohini Mohan Sahoo, permanent resident of Plot No. A-336, Sahid Nagar, Bhubaneswar, Dist. Khordha, Odisha, PIN-751007, have changed my name/surname from "Mahendra Sahoo'' to "Mahendranath Sahoo''. Henceforth, I shall be known as Mahendranath Sahoo for all purposes.
