By virtue of an affidavit sworn before the Notary Public, Cuttack on the 9th May 2024, I, SK Mustak Alli, S/o SK Asaraf Alli, Permanent residence of Vill. Karadapalli, P.O. Tigiria, P.S. Tigiria, Dist. Cuttack, Odisha have changed my name/surname from "S K Mustak Alli'' to "SK Mustak Alli''. Henceforth, I shall be known as SK Mustak Alli for all purposes.
