By virtue of an affidavit before the Executive Magistrate, dated the 7th December 2023, I, Smt. Sagupta Parween of Plot No. 200, Road-8, Unit-9, Saheed Nagar, Bhubaneswar declare that, I have changed my name from "Anita Lakra '' to "Sagupta Parween''. Hence for I shall be known as Sagupta Parween for all purposes.
