By virtue of an affidavit sworn before Executive Magistrate, Bhubaneswar on the 21st February 2025, I, Sumit Nayak, S/o Braja Bihari Nayak, resident of at LIG- 103, Nandan Enclave, Nandan Vihar, P.O. KIIT, Kalarahanga, P. S. Infocity, Bhubaneswar, Dist. Khordha, Odisha-751024 declare that, I have changed my name from "Saraswati Nayak'' to "Sumit Nayak'' & gender from female to male. Henceforth, I shall be known as Sumit Nayak & my gender shall be male for all purposes.
