But my daughter's name has been wrongly entered in my Family Ration Card B. No. 167026 as 'Sorna'. My daughter's original name is 'Rajeswari'. Her name has been wrongly entered as 'Sorna' in our Family Ration Card.

I make this affidavit in lieu of proof of my first female daughter's correct name as 'Rajeswari' but not as 'Sorna' as mentioned in the Family Ration Card of us and to be produced before the Directorate of Civil Supplies and Consumer Affairs, Puducherry for making necessary corrections in our Family Ration Card.
