I state that my registered name is 'Valarmathi'. The said statement of mine is substantiated through my Certificate of Birth, issued by the Registrar of Births and Deaths, Ariankuppam Commune Panchayat, Puducherry.
I further state that though my registered name is 'Valarmathi', my call name is 'Pallavi'. As such in my Family Ration Card under No. 31500, issued by the Department of Civil Supplies and Consumer Affairs, Puducherry my name has been mentioned as 'Pallavi' instead of 'Valarmathi'.
