I state that I was born to my parents Parimelazhagan and Mageswari on 17-8-1986 at Government Hospital, Tindivanam. My name is 'Preethi', but in all my records my name has been mentioned along with my father's name, namely Parimelazhagan. My name 'Preethi' has been mentioned only in my Family Ration Card.
CORRIGENDUM
