My name is 'Gandhimarie'. My husband's name is Arokiyasamy. In my birth and marriage record and my son Antoniraj's birth record my name is mentioned as 'Gandhimarie'. But in my Voter's Identity Card and Family Ration Card, my name is mispelled as 'Karoline'. I hereby declare that the names 'Gandhimarie' and 'Karoline' as found differently in the documents spoken to above refer to myself alone and none else.
In my son Antoniraj's Birth Certificate my husband's name is 'Arokiassamy' and in the Birth Certificate of my husband, his name is entered as 'Aroquiassamy' whereas, in his death record his I was born on 13-2-1964 at Karayambuthur Village, Bahour Commune, Puducherry to my parents Govindassamy and Calemany and the same has been duly registered in the Bahour Commune Panchayat. My actual name is 'Sinivassane' and the same has been found in my Birth Certificate, Family Ration Card, Aadhaar Card, Electoral Identity Card, etc. I have studied in the Government High School, Karayambuthur. In my Transfer Certificate issued by the said school authority; my name has been mentioned as 'Srinivasan' instead of correctly mentioning as 'Sinivassane'. In the above matter spelling mistake was caused by the said school authority. I affirm that the above names 'Sinivassane' and 'Srinivasan' refer to one and the very same person i.e., myself. I have been always using and signing my name only as 'Sinivassane'.
