I hereby declare that hereinafter my daughter will be called and sign in all documents and school records as 'S. Abhirami' instead of 'Abhirami Sathyajyothi'.

I hereby declare that I hereinafter use my daughter's name as 'S. Abhirami' in all her school records and all other official records relating to my daughter.
