The other name is as 'Z. Allaudeen @ Cheik Houssain' as mentioned in my Marriage Contract Deed of my marriage celebrated on 27-2-2003 with my wife Minhaj Begam at Kottakuppam, Vanur Taluk, Tamil Nadu. As such, I bear three names one is 'Z. Allaudeen', the other is 'Cheik Houssain' and the third is as 'Z. Allaudeen @ Cheik Houssain'.

That I have now changed my name from this 3rd day of February of the year 2014 onwards, retaining the name as 'Z. Allaudeen' and abandoning and relinquishing the other two names as 'Cheik Houssain' and 'Z. Allaudeen @ Cheik Houssain' once for all.

I would henceforth be known and called by my retained name as 'Z. Allaudeen' for all purposes.
