In my Family Ration Card issued by Department of Civil Supplies and Consumer Affairs, my name was spelled as 'Mohamed Shahul Hamid' (in Tamil). At present I am working at Puducherry Municipality as Revenue Inspector, in my Identity Card issued by Puducherry Municipality, my name is appeared as 'Mohamed Shahul Hameed'. To avoid confusion in different spelling in my name in the abovesaid documents, I hereby declare on oath that hereinafter I shall be called, spelled and signed as 'Mohamad Shahul Hamid'.
