By this deed I, the undersigned K.R. Mathivanan ( n e w n a m e ) , p r e v i o u s l y c a l l e d M a t h i v a n a n . R (old name), resident of No. 165/3, Abirami Colony, Main Road, Kottucherry, Karaikal-609 609, Puducherry State solemnly declare:

1. That for and on behalf of myself and my wife, children and remitter issue, I wholly renounce/ relinquish and abandon the use of my former name/ surname of 'Mathivanan. R' and in place thereof, I do hereby assume from this date the name/surname o f ' K . R .
