That my correct name is 'Vidjeacoumar' as per my Birth Certificate, my son's Birth Certificate, my son's Aadhaar Card and Voter's Identity Card, etc. But, my name is wrongly mentioned in my son's Driving Licence issued by the Transport Department, Puducherry (D. L. No. 20054336) as 'Vijayakumar'.
I hereby declare that the names 'Vidjeacoumar' and 'Vijayakumar' are one and the same person i.e., myself only.
