I am the deponent herein and I state that my father's name 'Paquiry' which is found in his Birth Certificate registered in the Nedungadu Commune Panchayat bearing Registration No. 125 and his another name 'Packirisamy', which is found in his Death Certificate registered in the Nedungadu Commune Panchayat bearing Registration No. 45 and in my Birth Certificate registered in the Nedungadu Commune Panchayat bearing Registration No. N/1985/00194 and LGR Patta bearing Patta No. 244/84 issued by Office of the Deputy Collector (Revenue), Karaikal and his name is entered in the same LGR Patta after his death as (late) Mani @ Packirisamy refer to one and the same person of my father only.
