The objections so received within the specified time will be considered by the Karaikal Municipality in accordance with the rules.

I, Vettrivel, son of Muthu, Hindu, aged 30 years and residing at the house bearing Door No. 7, Veeramamunivar Street, Radhakrishnan Nagar, Kathirkamam, Puducherry-605 009, do hereby solemnly affirm, to whomsoever it may concern as follows:

I am the deponent herein. I am well aware of the facts and the statement of my affidavit. I swear this affidavit to state that the names 'Vettrivel', 'M. Vettrivel', 'Vettrivel M Sengunthar', 'Vetrivel' and 'Vettrivel. M' are refer, identify, and relate to one and the same person, that is myself only.

I state that my registered name is 'Vettrivel', the said statement of mine is substantiated through the Certificate of Birth issued by the Registrar of Births and Deaths, Oulgaret Municipality, registered with it under Registration No. L/1984/231. I further state that though my registered name is 'Vettrivel' in single, along with my initial my name is 'M. Vettrivel' as such in my Aadhaar Card under No. 4099 2362 7925, my name has been mentioned as 'M. Vettrivel'.

Further, in my PAN Card No. BONPS3560P, my name has been mentioned as 'Vettrivel M Sengunthar'. I further state that in my academic certificates, my name has been mentioned as 'Vettrivel. M'.

That I have changed my name from 'Kalmeena Mary' to new name of 'Afreena'. Hereafter, I will be called and referred as 'Afreena' only. Whereas, in my Elector 's Photo Identity Card No. JTP0545756, my name has been mentioned with spelling as 'Vetrivel'. Similarly in my Family Ration Card under No. 075105, my name has been mentioned as 'Vettrivel' in Tamil language.
