TENDER NOTICE

Sealed tenders are invited for the sale of condumed mortuary chamber belonging to the Office of the Deputy Director, Government General Hospital, Yanam by the undersigned.

2. Tenders should be sent in sealed cover superscribed as "Tender for the purchase of mortuary chamber" addressed to the Deputy Director, Government General Hospital, Yanam-533 464, on or before 14-7-2015 at 3.00 p.m. and they wi l l b e o p e n e d o n t h e s a m e d a y at 4.00 p.m. in the presence of the tenderers. In case, if the prescribed date happens to be a holiday, the tenders will be opened on the immediate next working day.

3. The intending tenderers should enclose the demand draft/banker's cheque in favour of the Deputy Director, Government General Hospital, Yanam for an amount of ` 1,000 (Rupees one thousand only) along with their tenders. The earnest money deposit of unsuccessful tenderers will be refunded after the tender is over. The tender without earnest money deposit will be summarily rejected.

4. The tender received after the due date and time 5. The tenderer who has quoted highest rate will be considered as successful bidder. If the highest rate quoted by the tenderer is not satisfied by the undersigned, then open auction will be conducted on the same day. In the open auction, tenderers who have submitted their sealed tenders will only be allowed.

6. The successful tenderer should pay the entire amount soon after completion of auction including sales tax as applicable under the rules in force and clear the mortuary chamber immediately on the same day before 5.00 p.m. from the Office of the Deputy Director, Government General Hospital, Yanam attached to at it at their own cost and risk, failing which the Deputy Director, Government General Hospital, Yanam shall not be held responsible for either safe custody or loss or damage of the article once the auction is over.

7. No claim about the quantity and quality of the condemned article will be admitted once the tender is finalised.

8. The intending tenderers can inspect the condemned article during hospital working hours with prior permission of the concerned Head /Medical Officer in-charge of the Government General Hospital, Yanam.

9. The undersigned reserves the right to accept or reject any tender in part or whole without assigning any reason thereof. Auction is subject to discretion of the undersigned whose decision will be final in case of any dispute.

I, Dayalamourty alias Dhayalan, son of Natesan, Hindu, aged about 53 years, residing at No. 2, Mariamman Koil Street, Pangur, Puducherry, do hereby solemnly affirm and state as follows:

I state that I am the deponent herein and I am well aware of this affirmation.

I state and declare that I was born on 13-3-1962 at Pangur, Puducherry and my birth was duly registered before the Villianur Commune Panchayat, Puducherry under Registration No. M/1962/00048, dated 19-3-1962 and at the time of my birth, I had been named as 'Dayalamourty' and the same was entered in to my Birth Certificate.

I state and declare that apart from the said birth name, I had also been called by one nick name i.e., 'Dhayalan' and the same is entered into my Aadhaar Card, Voter's Identity Card, Family Ration Card and Smart Card issued by the Government of Puducherry.

I state and declare that the said two names are referring to one and the same person i.e., myself i.e., 'Dhayalan'.

I state that in his Birth Certificate and his Aadhaar Card No. 5678 0258 4361 issued by the Unique Identification Authority of India, his name has been mentioned as 'T. Ashokraj' and now the abovesaid name shall be changed as 'T. Thamizhkumaran' as per numerology and also I shall use always his name as 'T. Thamizhkumaran in all purpose in future.

I state that my son's correct name and his name henceforth be known as 'T. Thamizhkumaran' only.
