I state that I am the deponent herein and I am well aware of the facts of this deposition.

I state that my son namely 'V.M. Vishal Ajay' was born to us i.e., N. Vijaymurugan and V.M. Latha on 28-5-2007 at Government Maternity Hospital, Puducherry which was duly registered at Puducherry Municipality under Registration No. M/2007/07939, dated 30-5-2007.

Hence this sworn-in affirmation may kindly be accepted to this effect.
