Hence, I further declare that both the names of mine 'Selvi' and 'Radha @ Selvi' refer to one and the same person i.e., myself only.
I further declare that henceforth, I shall follow I, Selvi, wife of Venugopal, Hindu, aged 32 years, residing at No. 32, Sagayamadha Pada Salai, Govindasalai, Puducherry, do hereby solemnly affirm and declare as under:
