NOTIFICATION
In exercise of the powers conferred by sub-section (1) of section 30 of the Registration of Births and Deaths Act, 1969 (Central Act No. 18 of 1969) and with the approval of the Government of India, Ministry of Home Affairs, Office of the Registrar-General of India at New Delhi, the Lieutenant-Governor, Puducherry is pleased to make the following amendments to the Puducherry Registration of Births and Deaths Rules, 1 9 9 9 no tified vid e G.O. Ms. No .1 7 2 /LAS/9 9 -2 0 0 0 , d ated 29th December 1999 and published in Part-II of the Extraordinary Gazette No. 53, dated 30th December 1999, namely:—

AMENDMENT
1. Short title and commencement.— (i) These rules may be called
as the Puducherry Registration of Births and Deaths (Amendment)
Rules, 2015.
(b) They shall come into force with immediate effect.
2. Amendment in Form No. 1, Form No. 1A, Form No. 2 and
Form No. 3 under rule 5.— In the Puducherry Registration of Births
and Deaths Rules, 1969 (hereinafter referred to as the said rules) for the
existing columns 'Name of Father', 'Name of Mother' in Form Nos. 1. 1A, 3
and for the existing column (2) 'Name of deceased' and (5) 'Father/Mother/
Husband name' in Form No. 2 the following shall be substituted
respectively, namely:—
(a) In Form No. 1:
"4. Name of Father."
(Full name as usually written)
"UID No. of Father (if any)."
"5. Name of Mother."
"UID No. of Mother (if any)."
(b) In Form No. 1A:

(c) In Form No. 2:
"2. Name of the Deceased"
(Full name as usually written)
"UID No. of the Deceased (if any)."
"5(a). Name of Father."
"UID No. of Father (if any)."
"5(b). Name of Mother."
"UID No. of Mother (if any)."
"5(c). Name of Husband/Wife."
"UID No. of Husband/Wife (if any)."
(d) In Form No. 3:
"3. Name of Father."

3. Amendment in Form No. 5 and 6 under rule 8.— In the said rules, in Form No. 5 and 6 prescribed under section 12 (17), for the existing columns 'Name of Father' and 'Name of Mother', in Form No.5 and for the existing columns 'Name' and 'Name of Father/Mother/ Husband name' in Form No. 6, the following shall be substituted respectively, namely:—
(a) In Form No. 5:
Name of Father: UID No. of Father (if any).
Name of Mother: UID No. of Mother (if any).
(b) In Form No. 6:
Name: UID No. of the Deceased (if any).
Name of Mother: UID No. of Mother (if any).
Name of Father: UID No. of Father (if any).
Name of Husband/Wife: UID No. of Husband/Wife (if any).
