I, Mubarak Ali, son of Abdul Jaleel and, father and guardian of Selvan Ashif Ali (minor aged 15 years, date of birth 3-11-2001) Muslim, aged about 48 years and residing at No. 31, Pudu Mettu Street, Sulthanpet and Post, Villianur Commune, Puducherry-605 110, do hereby solemnly and sincerely affirm on oath and declare as follows:

That I am the deponent herein and I am well acquainted with the facts herein;

That my son is originally named as 'Ashif Ali ()' and the same is mentioned in my son's Birth Certificate bearing Registration No. 4097/ 2001/04, dated 19-11-2001 and in his Aadhaar Card bearing No. 9120 1542 0327. Whereas, in my Family Ration Card issued by the Department of Civil Supplies and Consumer Affairs, Puducherry bearing No. 187040, my son's home-calling name is entered as ' (Ahamed Ali)'.

I declare that all the abovesaid my son's names 'Ashif Ali ()' and '(Ahamed Ali)' are belong and refer to my said son only.

Further, I declare and state that hereinafter, my said son's name will be called and known only by his Birth Registration name as 'Ashif Ali ' and not by ' (Ahamed Ali)'.

Further, I declare that all the names 'Miraj Sulthania (•) and '• (Sulthana)' are belong and refer to myself only.

Further, I declare and state that hereinafter, I shall be called and known only by my Birth Registration name as 'Miraj Sulthania (•)' and not by 'Sulthania (•)'.

The above deposition is true and correct to best of my knowledge and belief.
