The objections so received within the specified period will be consider ed by this Commune Panchayat.
CHANGE OF NAME
I, Kasthuri, aged 30 years, daughter of Jayaraman and Rukumani, residing at No. 41, Reddiyar Street, Korkadu, Puducherry-605 110, solemnly declare that my name is written as 'Kasthuri' in my Birth Certificate in No. K/1987/26 and in my Elector's Identity Card under No. KTY0241588, in my Aadhaar Card No. 3985 3515 0713 as 'S. Kasthuri' in my India Driving Licence No. PY01 20090009910 and in my S.S.L.C. Mark sheet No . SEC 2310162/2 0 0 2 as 'Kasthuri J'. The abovesaid names 'Kasthuri', 'S. Kasthuri' and 'Kasthuri J' are refer to one and the same person i.e., myself. I have decided to change my name as per numer olo gy and that hereafter, I shall be called as 'Kasthuri Jayaraman'.
