In my Voter's Identity Card under No. KVY0410290 and in my Aadhaar Card under No. 3527 6683 1476 and in my daughter Jyotsna Krishnan's Certificate of Birth under Registration No. C/1999/04043 and in my Passport under No. J5234218, my name has been mentioned as 'Radha'.
In my Permanent Account Number (PAN) Card under No. AEFPR8686C, my name has been mentioned as 'Krishnan Radha'.
Further, I state that in the Family Ration Card under No. 027211 issued to my father-in-law, my name has been mentioned as ''.
Therefore, I do hereby state that all the said names are refer, identify and relate to one and the same person that is myself the deponent herein.
Therefore, I intend to change my name as 'Radha Krishnan' I shall henceforth be called and known by the name 'Radha Krishnan' only for all purposes.
I state that what are all stated in the above paragraphs is true and correct to the best of my knowledge, belief and information.
