My name is shown as 'Annalakshmi' in my Birth Certificate with Registration No. K/1984/ 0623 a n d Vo t e r 's I d e n t i t y C a r d N o . (Hajira Fathima)' and the names of my aforesaid three sons 'Premkumar', 'Kishore' and 'Shyam' as 'Abdul Bathin) ', ' (Mohamed Kaisan)' and ' (Mohamed Shamil)' respectively, consequent on our voluntary conversion of religion from Christianity to Islam on 18-10-2015 in the presence of the Government Regional Kazi, N.S. Abdul Hameed Maricar of Karaikal.
For and on behalf of myself and my aforesaid three Children, I wholly renounced, relinquished and abandoned the use of my former name 'Annalakshmi @ Kavitha' and the use of former names of my aforesaid three sons namely, 'Premkumar', 'Kishore', 'Shyam' and in places thereof assumed the names 'Hajira Fathima', 'Abdul Bathin', 'Mohamed Kaisan' and 'Mohamed Shamil' respectively so, that I and my aforesaid three sons may thereafter be called, known and distinguished not by our former names, 'Annalakshmi @ Kavitha', 'Premkumar', 'Kishore'
and 'Shyam' but, by our assumed and adopted names, 'Hajira Fathima', 'Abdul Bathin', 'Mohamed Kaisan' and 'Mohamed Shamil' respectively.
Further, I declare that we shall at all times hereafter in all records, deeds and writings and in all proceedings, dealings and transactions, private as well as public, and upon all occasions whatsoever use and sign the names of 'Hajira Fathima', 'Abdul Bathin', 'Mohamed Kaisan' and 'Mohamed Shamil' respectively as our names in place of and in substitution for our former names aforementioned.
