I hereby declare that I was converted myself to Islamic religion from Hinduism with the name of 'K. Abdul Rahman'.
Hereinafter, I shall be known as 'Abdul Rahman' and called and signed by Muslim name as 'Abdul Rahman'. Hence, this affidavit is made to prove my conversion and change of name.
