S' as found in my Military Disability Pension Paper, Military Canteen Card and Aadhaar Card and the name 'Rathinam' as found in the Voter's Identity Card, Family Ration Card, Bank Passbook and Old Age Pension Paper of the Government of Puducherry are identify and refer to me. Both the names are denote myself and I have been called my ho me as 'Rathinam'. I state that henceforth I will be referred to and identified as 'Rathinavelu. S' in all my official papers including my Voter 's Identity Card, Family Ration Card, Bank Passbook and Old Age Pension paper of the Government of Puducherry.
