I was a Hindu by birth, My birth name was 'Hari Priya', recently, I had embraced Islamic faith. Upon conversion of religion, I had renounced my birth name as 'Hari Priya' and assumed to myself the Muslim name 'Aysha ()'.
I hereby declare that I shall henceforth be known, called, referred and identified only as 'Aysha ()'.

S' in my Military Disability Pension Paper and Military Canteen Card as well as in my Aadhaar Card. I state that in my Voter 's Identity Card, Family Ration Card, Indian Overseas Bank Passbook and Old Age Pension Paper of the Government of Puducherry, my name is recorded as 'Rathinam'. The names 'Rathinavelu. S' as found in my Military Disability Pension Paper, Military Canteen Card and Aadhaar Card and the name 'Rathinam' as found in the Voter's Identity Card, Family Ration Card, Bank Passbook and Old Age Pension Paper of the Government of Puducherry are identify and refer to me. Both the names are denote myself and I have been called my ho me as 'Rathinam'. I state that henceforth I will be referred to and identified as 'Rathinavelu. S' in all my official papers including my Voter 's Identity Card, Family Ration Card, Bank Passbook and Old Age Pension paper of the Government of Puducherry.
