Further, I submit that my daughter 's name 'Soorya' as mentioned in her Birth Certificate, Passport, Aadhaar Card and in the Family Ration Card, and her name 'Soorya Manimaran' as mentioned in the 10th Mark Sheet issued by Central Board of Secondary Education, Delhi are refer to one and the same person i.e., my minor daughter only.

4. Further, I declare that she may be called only by the name 'Soorya' in future for all purpose and record.
